La fiesta continúa; los abuelos también bailan superando los achaques propios de su edad mientras los niños juegan. Las marionetas Colombina, Arlequino y Saracinos inician su baile. Todos están muy emocionados cuando el mago se quita su máscara: es Drosselmeier. Clara y Fritz, los hijos de Shtalbaun le piden a Drosselmeier que les regale las marionetas pero éstas ya se las han llevado. En su lugar les regala un ridículo muñeco cascanueces. Fritz arrebata el muñeco a Clara y lo rompe. Ésta le consuela echándole en la cama. Los invitados abandonan la casa después del baile para finalizar la fiesta navideña. Escena 2 Ha anochecido y la habitación donde se encuentra el árbol adquiere un aire misterioso, casi milagroso. Asustada, Clara va a ver a su muñeco enfermo Cascanueces. Melancólica observa como un búho se convierte en Drosselmeier mientras la mira con una sonrisa burlona. A su señal, del suelo aparecen ratones encabezados por su Rey. Clara quiere huir pero las fuerzas la abandonan y sólo consigue acercarse al árbol que muy deprisa empieza a crecer, mientras todo a su alrededor cambia. Los soldados de plomo cobran vida y, encabezados por Cascanueces, inician una batalla contra los ratones. Las fuerzas son desiguales, van venciendo los ratones cuando Cascanueces se queda sólo ante el ejército de roedores. Clara lanza un zapato al Rey de los Ratones y consigue ahuyentarlos. Cascanueces yace en el suelo. Clara se acerca a él: es un joven príncipe. Él la mira agradeciendo su ayuda. Escena 3 La habitación se convierte en un bosque invernal. Unos levísimos copos de nieve bailan milagrosamente mientras una estrella del árbol atrapa a Clara y al Príncipe Cascanueces como en un sueño, introduciéndolos en una carroza que les conducirá hasta la cima del árbol. Acto II Escena 4 Juntos inician su viaje hacia la cima del árbol, pero los ratones les alcanzan entablándose una nueva batalla. En esta ocasión Cascanueces sale vencedor. Llegan al reino de los muñecos donde son recibidos por doce pajes. El Príncipe narra lo sucedido y cuenta como Clara le salvó, motivo por el que todos muestran su agradecimiento, celebrando una gran fiesta en honor de ambos. Se despiden del reino de las Nieves y siguen su camino por el Río de la Limonada hasta llegar al Reino de las Golosinas donde la Reina de los Dulces los espera con un agasajo en su honor en donde varios bailarines de distintas partes del mundo bailan para ellos. Escena 5 Finalmente, el mundo de ensueño de Clara se desvanece y ella se despierta dándose cuenta que todo ha sido un sueño.
