Cette molécule est maintenant omniprésente dans les médias notamment en raison du battage médiatique qui en est faite autour du professeur Raoult et son équipe de l’IHU Méditerranée Infection. Nous sommes ainsi confrontés à un théâtre médiatique où des déclarations sensationnalistes produisent en retour sur Internet le développement massif de théories du complot. La chloroquine serait-elle une solution simple au coronavirus (COVID-19), auquel le gouvernement ne nous donnerait pas accès? Il serait pourtant peu compliqué aux journalistes d’aborder ce sujet avec un peu de recul en regardant simplement l’état des connaissances scientifiques. En effet, la recherche n’a jamais été aussi réactive sur un sujet que celui du COVID-19, avec des publications quasi en temps réel. Elles sont par ailleurs en accès libre et elles donnent une vue assez précise, même pour un non spécialiste, de l’état de nos connaissances. Je vous propose ici une synthèse de la quinzaine d'articles scientifiques publiés sur la chloroquine et le COVID-19. Elles sont toutes référencées dans la base de données PubMed classiquement utilisée. L’objectif est de donner un peu de recul à ce que l’on peut entendre et je vous liste les liens vers les articles à la fin si vous souhaitez regarder en détail. Ainsi, quand on lit les articles, on découvre que de nombreux traitements potentiels existent et sont actuellement testés sur le COVID-19 [4, 9]. Ainsi, l’article [4] fait un point complet sur les connaissances liées à la sévérité du COVID-19 (durée d’incubation, manifestations cliniques, imagerie, traitement, évolution des patients). Concernant le traitement plusieurs molécules d’intérêt sont listées: ? lopinavir/ritonavir (Kaletra®), nucleoside analogs, neuraminidase inhibitors, remdesivir, umifenovir (arbidol), DNA synthesis inhibitors (such as tenofovir disoproxil, and lamivudine), chloroquine, ACE2-based peptides, 3C-like protease (3CLpro) inhibitors, novel vinylsulfone protease inhibitor, teicoplanin, and Chinese traditional medicine (such as ShuFengJieDu or Lianhuaqingwen capsules). ?. Dans [4 et 14] l’antiviral remdesivir est considéré comme le plus prometteur. - A partir d’études in vitro la chloroquine est également jugée intéressante [7, 12], mais des chercheurs appellent à rester prudents car les éléments tangibles sont faibles [5, 10] malgré des premiers résultats positifs sur des souris [15]. - Lorsque l’on compare les molécules de l’hydroxychloroquine (HCQ) par rapport à celui de la chloroquine (CQ), les mêmes effets positifs pourraient être produits par l’HCQ (en pre et post infection) qu’avec la CQ. L’HCQ a, par ailleurs, moins d’effets secondaires notamment pour les femmes enceintes, est plus facilement accessible [1, 2, 8] et également moins chère [13] De plus, dans [5], à partir de 23 études cliniques en cours et 6 articles les auteurs notent ainsi que la CQ est recommandée pour son acceptabilité et la sécurité des patients. Ils insistent toutefois en ciblant les recommandations de l’étude Chinoise [12] que ? les auteurs indiquent également que ces résultats [sur la chloroquine] sont obtenus sur ‘plus de 100 patients’ d’études. Selon ces auteurs, si c’est expérimental il faut une évaluation éthique précise. Au final cette question résume bien la problématique. La CQ/HCQ parait intéressante mais elle n’est pas seule et aucune donnée scientifique issue d’études cliniques n’est encore disponible pour corroborer son intérêt. L’équipe de Raoult indique dans [6] qu’ils recommandent la chloroquine pour combattre des maladies virales et cela… depuis 2007. Ainsi, cette équipe maîtrise bien la molécule et dispose d’une grande expertise sur celle-ci. Par contre, cette approche, locale, est bien expérimentale (au sens de non prouvée par des études) et les praticiens doivent nécessairement gérer les risques/bénéfices pour les patients. Enfin, il n’existe pas de preuves scientifiques pour en faire une démarche générale et le consensus scientifique semble être qu’il ne faut pas s’emballer sur la CQ/HCQ.
