• BestPracticeより：原文を読んでないが、どうも、フッ素入りの歯磨きで磨いて、ソフトからハードに変わる、inactive lesionsは、修復しないような感じであり、僕の臨床経験に合致する。

Within a period of 2-6 months all lesions had changed from soft, greasy and yellowish to leathery or hard, darkly discoloured tissue, indicating a gradual transition from active into inactive stages of caries. The observations stress the dynamic nature of the root caries process. It is concluded that when dealing with root surface caries it is essential to distinguish between active and inactive lesions and that classical operative treatment to a great extent can be avoided.

• ロンドンの施設居住平均80歳ぐらいの高齢者各５０名にフッ化物バーニッシュとクロルヘキシジンバーニッシュは、フッ化物バーニッシュとプラセボバーニッシュと比較して、根面う蝕の進行を抑制した。この論文は、ＲＣＴではなく脱落率が多かった。対照群では、有意な進行をみたとあるので、フッ素のバーニッシュのみでは、効果がないということになります。これは、困った。回数を増やす、濃度をあげる、洗口を併用するなどの論文はないのか？

• 本Wikiでは、基本的に臨床論文（臨床のアウトカム）を選んでいますが、S.mutansの量の変化がアウトカムですが、おもしろかったので紹介です。クインテッセンス2005Vol24No10p2221での紹介記事で知りました。
  • もともと、歯周病の初期治療によって、歯周病原的な細菌からう蝕病原的な細菌叢（フローラ）へ変わるということが示唆されていたようである。そして結果は、スケーリングだけでは、S.mutansが増えます。クロルヘキシジンのみでは、CHXを使用中は良いが中止すると増える。CHXを2ヶ月使ってからフッ素洗口を6ヶ月行った群は、S.mutansが劇的に減少したそうです。よって、歯周治療を行った後のう蝕発生に注意する必要があるそうです。また、LactobacilliやP.gingivalなどはも同じ傾向であったが、大きな差がないような感じである。

• 歯チャンネルで、ものすごく勉強されている先生に教えてもらいました。根面カリエスの変化についてですが、15名、99歯面の活動性根面カリエスに対して12ヶ月間の徹底したプラークコントロール（※間食指導、ブラッシング指導、PMTCなど）&フッ素を行った結果、近心15歯面→4歯面(27%)がinactiveに。同様に、遠心26→2(8%)、頬側46→25(54%)、舌側12→5(42%)。

  • The vast majority of trials of fluoridated toothpastes has been conducted on coronal caries in children and adolescents and little information is available on the effect of such toothpastes on root-surface caries in adults. Preliminary results of studies are promising, but more study is required.　「これまでフッ化物配合歯磨剤について行われた試験のほとんどは、小児と青少年の歯冠部の齲蝕に関するものであり、このフッ化物配合歯磨剤の成人の歯根面齲蝕への効果に関する報告はわずかしか無かった。」

• 根面カリエスの２次カリエスという大切な論文。これも、歯チャンネルで、ものすごく勉強されている先生に教えてもらいました。157歯面に発生した根面カリエスの部位についての観察研究によると、近心面 19%　遠心面 37%　頬側 37%　舌側 12%、セメントエナメル境(C-E Junction) 25%、修復物辺縁(Restoration margin) 51%、歯肉辺縁(Gingival margin) 7%、混合型(Complicata) 17%。・・ということで、根面カリエスの約7割（修復物辺縁＋混合型）は、修復物絡み、、みたいです。

• プラークコントロール・砂糖のコントロールやフッ素の応用を頑張れそう
