• Glass-ionomer修復が、AF、CR、CRインレーより2次カリエスが抑制できるという論文はなかったそうです。
  • これは、困った。でも、ハイリスクの論文は２つだし、RCTは３つだし、よくわかってないという感じですね。ごくごくわずか小さいという感じもしないでもない程度。

カリエスをどこまで除去するか？

• レジン研究者が、カリエスを若干残しても、しっかりと封鎖すればOKという場合に引用されると考えられる論文などを紹介してある論文。Freeでよめる。RCTは、以下の４本あるそうです（検索式は書いてなかった）。

• 上記のMertz-Fairhurst EJ論文について。
  • Conventional amalgam restorations.　
    • 柔らかいカリエスは取りました。We completely removed soft demineralized dentin and chalky white demineralized ename.　We did not remove dentin or enamel that was stained and hard.
    • 予防拡大をしました。将来カリエスになりそうな、noncarious fissuresまで伸ばした。
  • Conservative amalgam restorations.
    • 柔らかいカリエスは取りました。
    • 予防拡大は、しませんでした。the preparation was not extended into unaffected fissures and grooves.
    • アマルガムをおいた後、その上にシーラントをしました。After the amalgam was placed, the operator applied sealant over the restoration and all pits and fissures of the tooth.
  • Cariostatic sealed composite restorations.
    • もろいところは、すべて取ったけど、ベベルの下などは残した。We removed all of the crumbly, opaque demineralized enamel with a bur until we reached translucent sound enamel. We did not remove undermined enamel or caries below the bevel.
    • 間違って、すべて取ったら、それは除外。
    • Kiddの論文には、「DEJ not made caries free; moist, soft, infected dentine left at DEJ and over pulp; restored bonded, sealed, composite」とあるけど、over pulpは、取ったのでは？？
  • 問題点ではないが、あくまでも、しっかりシールされる、ClassIのカリエスである。
  • 問題点かな、８歳から５２歳ということで、そりゃ、離れすぎと思うが・・・。
  • １０年後：レジンと従来型AFは、同じであった。* U: clinical failure unrelated to the study.
    • レジングループ：12 + 4 U*(14%)† (n = 85)
    • シールAFグループ：1 + 1 U*(2%)† (n = 44)
    • 従来型AFグループ：7(17%)† (n = 41)
    • 77 CompS/C restorations vs. 77 sealed amalgam (AGS) restorations†→0.0322 (Significant difference;AGS is superior)
    • よって、シールしたAFが良いのなら、やはり、レジンでも、しっかり取っておいた方が良かったと言うことにはならないか？　もっとも、この論文の結論は、「we recommend that Class I amalgam restorations be sealed immediately after they have been placed.」です。

のこりの、Magnusson 1977ですが、1977年です。これは、原文を手に入れないと詳細不明ですが、1977年です（しつこい）。と調べていると、

• 実は、ごく一層残して、レジンなどでしっかりと封鎖すれば、良いという臨床研究の長期予後の論文を読んだことがありません。また、AIPCは、途上国でタービンなどが使えないところでと聞いたことがあります。図を2つ書きましたが、第3象牙質でしたね。深く考えた図じゃないので、気軽にみてください。私は、カリエスを残して、上から封鎖されると、中で、細菌がうじゃうじゃという気になるので（細菌が、どこまでいるかは別として、気分です）、やはり、カリカリっというところまで取ってほしいですね。でも、抜髄はやだし・・・、まだまだ勉強不足で、よくわかりません。　AIPC：　 http://wwwsoc.nii.ac.jp/jscd/data/whatsnew/iinkai6...
